Per-Member-Per-Month (PMPM) is based on actively enrolled members, not total employee headcount. Example: 50 employees eligible, 30 enroll = you pay for 30. Pricing tiers: $35 PMPM for 1-25 active members, $25 PMPM for 26-75 active, $15 PMPM for 76+. Implementation fee ($8K) is one-time and covers setup, training, and first-month delivery. Billing is monthly in arrears based on actual active member count (member must complete at least one check-in per month to count as "active").
